About Us

“ODR launched the Reentry Intensive Case Management Services (RICMS) program in April 2018 to provide care coordination and system navigation services to individuals who have been arrested for, charged with and/or convicted of a crime with mild to moderate mental health and substance use disorders. The program is delivered by peer mentors known as Community Health Workers (CHWs) with lived experience who are able to relate to and help their clients follow through with treatment.” – ODR’s 2019 Prop 47 2 Year Preliminary Report Ascent works diligently to improve the quality of life of each client we come into contact with by removing the burden they face when attempting to navigate social services As a reentry intensive case management service, Ascent’s CHWs collaborate within the Long Beach community and greater LA services to provide clients with not only social and economic support but also the emotional and psychological support we believe is the key to stability Add a footer 2 and long-term success.

August 2018

Ascent launches its reentry program and became one of LA County’s initial reentry-intensive case management service providers with Whole Person Care. Starting with just 2 Community Health Workers, we are now a dynamic team of 6 CHW’s and support staff.

August 2019

Ascent partners with local reentry providers and community members to start the Long Beach chapter of the Community Action Partners Alliance (Long Beach CAP Alliance).

June 2019

Through the City of Long Beach’s Justice Lab initiative, Ascent establishes a City Jail Partnership with a new referral process through an in-jail Clinician.

September 2019

Ascent hosts one of LA County’s 6 Community Engagement Workshops to provide feedback on the Alternatives to Incarceration recommendations.

The foundation of Ascent’s success comes from our dynamic team of Community Health Workers (CHW’s). With their wisdom and lived experience, CHW’s meet the clients where they are with empathy and understanding. Ascent’s multi-faceted team of CHW’s are guides of empowerment for our clients and help navigate clients through the obstacles of stabilization and re-integration with warm hands through:

  • Stabilization needs

  • Enrollment in social services

  • Physical and mental health

  • Employment and education

  • Cognitive behavioral intervention

  • Substance use disorder

Ascent & Advocacy

As part of our philosophy of community healing, Ascent is actively engaged throughout the year with different community projects. Within the last 2 years, Ascent has partnered with Alternatives to Incarceration (ATI), the Justice Lab (a multidisciplinary initiative to increase CJS and social services data sharing and end recidivism in the City of Long Beach) and Community Action Partners Alliance (CAP) to help empower the voices of citizens affected by incarceration and unite important stakeholders like Long Beach Health Department, the Long Beach Fire Department, Long Beach Police Department, and more. Together with these local partners, we aim to continue cultivating a more accepting and accessible reentry pathway for our clients.
